An index usually contains the titles or descriptions of first-level titles (chapters of longer works) and often includes second-level titles (sections or Title A) within chapters and occasionally third-level titles (subsections) or Title B) within sections. The level of detail in the tables of contents depends on the length of the work, with longer works showing a smaller number. Formal reports (ten or more pages and too long to be written in a memo or letter) also contain an index. In an English-language book, the table of contents usually appears after the title page, copyright notices, and in journals after the abstract and before lists of tables or figures, the preface and preface.

The printed indexes indicate the page numbers where each part begins, while digital links are provided for each part. The format and position of the page numbers are a matter of style for the publisher. When page numbers are displayed after the header text, they may be preceded by characters known as guidelines, usually dots or full stops beginning with the chapter or section headings on the opposite page, or the page numbers may be kept closer to the titles. In some cases, the page number is displayed before the text.

IUGR and preterm birth as risk factors for morbidity/mortality

CHERG has identified the need for additional work on intra-uterine growth restriction (IUGR) and pre-term birth. Both these conditions, individually and together, are important risk factors for neonatal (and perhaps later) mortality. Initially, CHERG examined the contribution of low birth weight at term (< 2500g) to neonatal mortality, and, in the latest iteration, estimates were made disaggregating the incidence of neonatal mortality by cause amongst newborns weighing 1500-1999g, and 2000-2499g, as opposed to weighing more than 2500g at term.  Deaths amongst newborns weighing less than 1500g were considered to be completely due to prematurity, and no deaths due to prematurity were estimated otherwise. There are recognized limitations to this approach, which was formulated within the strictures of the published data available. Information in this field is rapidly changing and new data are emerging that can be used to improve current estimates of the magnitude and distribution of the problem of IUGR and preterm birth, as well as their individual and combined contribution to neonatal and infant morbidity and mortality. Specifically, there are community-based studies where reasonably good estimates of gestational age are available that could now address this question more effectively. This type of information is important for program and research priority-setting, as it provides the basis for defining risk factors and for evaluating the impact of interventions to prevent these two different contributors to the mortality burden among the most vulnerable newborns.

There are 3 specific aims proposed for this activity:

  • Estimate global and region-specific incidence of preterm and IUGR
  • Estimate the disease burden (morbidity and mortality in neonates and infants) associated with IUGR and preterm globally and by region
  • Identify maternal risk factors, with emphasis on nutrition, for IUGR and preterm, and estimate the disease burden attributable to these conditions.

Methods and Expected Products

After suitable data sets are identified, an investigators’ collaborative group will be formed to plan the analyses. The investigators’ group will then reconvene to provide feedback on analyses and interpretation of results. The results will be published in a peer-reviewed journal and made publicly available.

Causes of maternal mortality

The overwhelming majority of maternal deaths occur in developing countries and arise from the risks attributable to pregnancy and childbirth as well as from the poor performance of health services. To reduce maternal deaths globally we need to assure that effective services are provided for improving overall maternal health. This task, in turn, requires targeted health care interventions that are informed by reliable and valid epidemiological data. A comprehensive summary of the magnitude and distribution of the causes of maternal deaths is therefore a critical component of a global strategy to reduce these deaths and, more broadly, to inform reproductive health policies and programs.

Methods and Expected Products

CHERG will conduct work related to causes of maternal deaths in three domains:

Estimates of causes of maternal deaths:  A systematic review conducted by WHO and published in 2006 in the Lancet provides a regional distribution of causes of maternal deaths. WHO is currently in the process of updating that review with the aim of providing up-to-date global/regional/sub-regional estimates, and will be collaborating with CHERG to finalize this effort.

Estimates of individual causes and contributory factors for maternal deaths:  Work in this domain aims to improve information on causes of maternal deaths by providing up-to-date estimates of incidence/ prevalence and case fatality rates of main causes of maternal deaths. Main causes of maternal deaths include, hemorrhage, pre-eclampsia/eclampsia, unsafe abortion, maternal sepsis, and indirect causes in some settings such as malaria. It is important to understand the scale and distribution of these conditions as well as case fatality rates to inform planning of interventions. Additionally, work will be conducted to understand the various contributing factors that do not directly or indirectly cause maternal deaths, but increase a woman’s risks of dying from a maternal cause, such as HIV/AIDS, anemia and obstructed labor.

Classification of maternal deaths:  The CHERG will contribute to the finalization of a classification system for identifying causes of maternal deaths that is being developed by RHR/HRP by supporting the write-up and dissemination of this effort.
